Surgeons can categorize breast types based on the position of the nipple relative to the breast crease as well as which direction the nipple is pointing. If your breasts have minimal sag, and your nipple is at or slightly below the inframammary fold, then implants alone might suffice. Otherwise, you may also need a mastopexy to maintain lasting results.
